Patna: In a landmark achievement showcasing advanced cancer care and surgical excellence, AIIMS Patna has successfully preserved the leg of a 25 year old patient diagnosed with a rare and aggressive bone cancer offering renewed hope for patients facing similar life altering conditions.
Makar Dhwaj, a resident of Deoria, Uttar Pradesh, had been experiencing persistent pain and swelling in his left leg for several months. Initially treated elsewhere as a routine fracture following minor trauma, his condition failed to improve.
When he presented at AIIMS Patna on May 13, 2025, doctors identified a pathological fracture of the tibia, raising suspicion of an underlying malignancy. Subsequent investigations confirmed Ewing Sarcoma, a rare and highly aggressive bone cancer that often necessitates amputation.
Rather than opting for conventional amputation, a multidisciplinary tumour board including experts from Orthopaedics, Radiation Oncology and Surgical Oncology devised a limb salvage strategy aimed at both curing the cancer and preserving function.
The patient underwent systematic chemotherapy from August 2025 to January 2026, which significantly reduced tumour burden and improved surgical feasibility.
On March 13, 2026, a highly specialized limb salvage surgery was performed. The cancer affected segment of the tibia was carefully removed along with surrounding tissue. In a cutting edge approach the excised bone was treated with high dose extracorporeal radiation (50 Gy) to eradicate residual cancer cells. The sterilized bone was then re-implanted into the patient’s leg. Structural stability was restored using titanium plates and screws and a muscle flap reconstruction was performed to enhance healing and functionality.
This advanced technique enabled complete oncological clearance while preserving the limb, avoiding the physical and psychological impact of amputation. The patient’s post-operative recovery was encouraging. Physiotherapy began on Day 2, emphasizing early mobilization. Within three weeks, he was discharged in stable condition. He regained the ability to walk with support, marking a significant milestone in recovery.
Medical experts note that amputation has traditionally been the standard treatment in such complex cases. However, this case highlights how advancements in surgical oncology, precise planning and collaborative care are transforming outcomes making limb preservation increasingly achievable.
