Can a Person Become a Father After Gender Transition? Bihar Couple’s Story Puts Spotlight on Medical Facts

The marriage of Rahul, who transitioned from female to male, and BPSC teacher Nayanshree has sparked conversations about gender dysphoria, hormone therapy, surgery, legal rights and the realities of parenthood after gender transition.

Jamui: A wedding in Bihar’s Jamui district has captured widespread public attention, not only because of the couple involved but also because it has opened up a broader conversation about gender identity, medical science and the realities of gender transition in India.

On May 31, 2026, Rahul, who was known as Rakhi Kumari until six months ago, married his cousin Nayanshree, a BPSC-appointed school teacher, in a traditional Hindu ceremony. The marriage quickly became a topic of discussion across social media and local communities, raising questions about gender transition, legal recognition, medical procedures and whether a transgender man can become a biological father.

While the couple is currently away on leave following their wedding and unavailable for comment, their story has brought renewed focus to a subject that remains widely misunderstood despite growing awareness.

A Journey Years in the Making

According to family members, Rahul’s journey did not begin with surgery. Rather, it was rooted in feelings and experiences that stretched back to childhood.

Rahul’s father, Narayan Das, said his child preferred boys’ clothing and activities from an early age. As the years passed, he noticed increasingly masculine traits in behavior, mannerisms and physical development.

Family members say Rahul often felt uncomfortable with the gender assigned at birth. During his graduation studies in Patna, he reportedly began consulting medical professionals to better understand his feelings and options.

After researching available treatments, Rahul chose to undergo a gender transition process under medical supervision. In early 2025, he reportedly began psychological counselling and assessment, followed by hormone therapy in Delhi.

The hormone treatment resulted in visible physical changes, including a deeper voice and facial hair growth. In November 2025, Rahul underwent gender-affirming surgery at Delhi’s AIIMS. Following the procedure, he remained under medical observation for several months and attended regular follow-up consultations before eventually returning to normal life.

Six months later, he married Nayanshree, whom family members describe as a childhood friend and long-time companion.

Days after the wedding, Rahul addressed the public through social media, saying, “This is our life, and we will live it on our own terms.”

What Is Gender Transition?

The marriage has prompted widespread curiosity about gender transition and the medical processes involved.

Dr. Suryanandan, a physician from Jamui, explained that gender transition is not a sudden event but a structured and often lengthy process involving medical, psychological and legal considerations.

“Gender transition generally begins with detailed psychological counselling and assessment,” he said. “Medical experts first determine whether the individual is experiencing persistent gender dysphoria and whether they fully understand the implications of transition.”

Gender dysphoria refers to the distress some individuals experience when their gender identity differs from the sex assigned to them at birth.

Medical professionals emphasize that not everyone questioning their gender undergoes transition. The process typically follows extensive evaluation by mental health specialists before any medical intervention is considered.

The Role of Hormone Therapy

Hormone therapy is often one of the most significant stages of transition.

For individuals transitioning from female to male, testosterone is prescribed under medical supervision. Over time, this can lead to a deeper voice, increased facial and body hair, changes in muscle mass and a more traditionally masculine appearance.

According to doctors, these changes may begin within months but can continue developing for one to two years or longer.

Many patients require long-term hormone therapy and regular monitoring to ensure safety and manage potential side effects.

Medical experts caution that hormone medications should never be taken without professional supervision.

Understanding Gender-Affirming Surgery

Gender-affirming surgery, sometimes referred to as sex reassignment surgery, encompasses a range of procedures designed to align an individual’s physical characteristics with their gender identity.

Specialists say the process may involve multiple stages and can vary significantly depending on individual goals and medical needs.

Dr. Ashwini Das, a plastic surgeon who performs such procedures, noted that complete transition may involve numerous surgical and non-surgical interventions affecting physical appearance, hormonal profile and reproductive anatomy.

The overall process can take several years and may cost anywhere from a few lakh rupees to more than ₹10 lakh, depending on the procedures chosen.

However, many individuals opt for only selected treatments rather than pursuing every available intervention.

Can a Transgender Man Become a Biological Father?

One of the most frequently asked questions following Rahul’s marriage concerns parenthood.

Medical experts explain that while gender-affirming surgery and hormone therapy can alter physical characteristics and gender expression, they do not create reproductive capabilities associated with the opposite biological sex.

According to specialists, a transgender man who was assigned female at birth does not produce sperm and therefore cannot biologically father a child in the conventional sense.

However, family-building options such as assisted reproductive technologies, including IVF and surrogacy, may be available depending on individual circumstances and prior fertility preservation measures.

Similarly, transgender women do not develop the biological ability to menstruate or carry a pregnancy.

Experts stress that reproductive possibilities vary from person to person and should be discussed with qualified fertility specialists before undergoing treatment.

Who Is Eligible for Gender Transition?

Mental health professionals say the process is generally reserved for adults diagnosed with gender dysphoria following comprehensive evaluation.

The assessment seeks to determine whether the individual’s gender identity concerns are persistent and deeply rooted rather than temporary or situational.

In India, medical practitioners generally require mental health clearance before proceeding with major interventions. Patients are also informed about the benefits, risks, long-term commitments and social implications of transition.

Many experts consider adulthood an important milestone because individuals are better positioned to provide informed consent regarding life-altering medical decisions.

Is the Procedure Safe?

According to medical specialists, gender-affirming procedures are generally considered safe when performed by qualified professionals and after appropriate medical evaluation.

Before surgery, patients typically undergo extensive testing, including assessments of heart health, liver and kidney function, blood sugar levels and overall physical fitness.

As with any major surgery, risks can exist, but doctors say serious complications are uncommon when established medical protocols are followed.

What Does Indian Law Say?

India’s legal framework has evolved significantly regarding gender identity and personal autonomy.

Legal experts note that an adult’s decision to undergo gender transition is protected under constitutional principles relating to privacy, dignity and personal liberty.

The Supreme Court’s landmark judgments on privacy and gender identity have reinforced an individual’s right to make decisions concerning their body and identity.

Medical institutions generally require informed consent documentation and psychological evaluation before major gender-affirming procedures are undertaken.

Beyond the Headlines

For many observers, Rahul and Nayanshree’s wedding is more than a personal milestone. It has become a catalyst for discussions about identity, acceptance, medical science and evolving social attitudes in India.

While debates continue online and in public spaces, experts say one takeaway remains clear: gender transition is neither a quick decision nor a simple medical procedure. It is a complex journey involving personal conviction, professional guidance and long-term commitment.

As awareness grows, specialists hope that public conversations will increasingly be informed by medical facts, legal understanding and empathy for those navigating questions of identity and belonging.
